Flagstar Rehab is seeking an Occupational Therapy Assistant (OTA) for skilled nursing facilities in New York. This role offers flexible scheduling, with opportunities across Full-Time, Part-Time, Per Diem, including weekends and evenings, to fit your lifestyle while delivering patient-centered care.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Requirements and Responsibilities:

• Current NYS OTA License and Registration
• Valid BLS Certification
• EMR Competence (Proficiency in REHAB OPTIMA + POINT CLICK CARE a plus!)
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
