Job summary

Flagstar Rehab is seeking Occupational Therapy Assistants (OTA) to provide skilled rehab services in SNFs across New York. This role requires active NYS OTA license and BLS certification. We offer flexible scheduling and comprehensive onboarding to support your clinical goals.

Earn hourly pay with options for W-2 ($34–$36) or 1099 ($38–$40), plus weekly payments and a range of benefits depending on assignment. Join a team that values patient care and clinician growth.

Qualifications

• Current NYS OTA License and Registration
• Valid BLS Certification
• EMR Competence (REHAB OPTIMA + POINT CLICK CARE a plus!)
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
