Flagstar Rehab is seeking Occupational Therapy Assistants (OTAs) for Skilled Nursing Facilities. We offer a flexible “choose the way you earn” model and remote onboarding to get you into the role quickly.

Competitive hourly pay and rapid interviews support our mission to connect compassionate OTAs with geriatric facilities. We provide opportunities across Full-Time, Part-Time, and Per Diem roles with weekly pay, paid orientation, and multiple insurance options.

Formación

• Current NYS OTA License and Registration.
• Valid BLS Certification.
• EMR proficiency, REHAB OPTIMA/POINT CLICK CARE a plus.
• Meets all State and Federal requirements for US employment in a Nursing Facility.

Responsabilidades

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to SNF patients with conditions such as TBI, CVA, Stroke, Parkinson’s, MS, cancer, orthopedic surgery, and other diagnoses.
• Monitor and document patient progress.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)
Occupational Therapy Assistant Requirements and Responsibilities:

• Current NYS OTA License and Registration
• Valid BLS Certification
• EMR Competence (Proficiency in REHAB OPTIMA + POINT CLICK CARE a plus!)
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress
