Job summary

Flagstar Rehab is seeking licensed Occupational Therapists (OT) to join Skilled Nursing Facilities across New York. We offer flexible assignments, a “choose the way you earn” model, and support in setting up a corporation if you prefer independent work.

OTs manage evaluations, develop individualized treatment plans, and provide therapy in SNFs for aging populations. Remote onboarding and weekly pay help maintain work-life balance.

Qualifications

• Current NYS OT License and Registration.
• Valid BLS Certification.
• EMR software proficiency.
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under supervision.
• Provide quality therapy services to SNF residents with TBI, CVA, Stroke, Parkinson’s, MS, cancer, orthopedic surgery, and other conditions.
• Monitor and document patient progress with EMR software.

Pay Rate:

$46-$48 Hourly (W-2), $57-$60 Hourly (1099)

Occupational Therapist Requirements and Responsibilities:

• Current NYS OT License and Registration
• Valid BLS Certification
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
• Malpractice/Liability Insurance (if applicable)
• Provide efficient evaluations and create individualized treatment plans for COTAs to follow under your supervision.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress with the use of innovative EMR software.
