Job summary

Flagstar Rehab is seeking Occupational Therapy Assistants (OTA) for Skilled Nursing Facilities in New York. This role supports patient care in SNFs with opportunities for full-time, part-time, per diem, weekends and various shifts.

Requirements include active NYS OTA license, valid BLS, and proficiency with EMR systems (REHAB OPTIMA + POINT CLICK CARE a plus). Enjoy weekly pay, benefits options, and a supportive team environment.

Qualifications

• Current NYS OTA License and Registration.
• Valid BLS Certification.
• EMR Competence (REHAB OPTIMA + POINT CLICK CARE a plus!).
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson's, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnosis.
• Monitor and document patient progress.

Hourly Pay Rate: $34-$36 Hourly (W-2), $38-$40 Hourly (1099)

Occupational Therapy Assistant Requirements and Responsibilities:
