Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant to join our in-house skilled nursing facility team in Le Center, MN. You will plan, implement, and monitor therapy services under supervision, ensuring documentation aligns with CMS standards and the resident's plan of care.

Qualifications

• Must hold active OTA licensure in state(s) of practice.
• Associate's degree in Occupational Therapy.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine the best intervention.
• Perform treatment plans and follow short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents and families.
• Establish resident therapy schedules in coordination with team members.
• Conduct therapy according to treatment plans and ensure proper documentation and billing.
• Perform all other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards of practice daily.
