Job summary

Evolve Therapy Services, LLC is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Minneapolis, MN. The role involves planning, organizing, and directing Occupational Therapy Services in the facility to maintain high-quality resident care under supervision of a licensed therapist.

Qualifications

• Associate degree in Occupational Therapy required.
• Active OTA licensure in state(s) of practice.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Conduct regular screening of residents to determine best intervention per patient.
• Perform treatment plans following established goals.
• Interpret and communicate evaluation findings to residents, families, and inter-disciplinary team.
• Establish individual resident therapy schedules with team members.
• Maintain proper documentation and billing of occupational therapy services.
• Perform other duties as assigned to effectively treat patients.
• Comply with CMS documentation standards daily.
