Job summary

Evolve Therapy Services is seeking a Certified Occupational Therapy Assistant for an in-house Skilled Nursing Facility in Minneapolis, MN. Full Time, Part Time, or PRN positions available.

The OTA will plan, develop, and direct occupational therapy services to ensure the highest quality resident care. Responsibilities include screenings, treatment planning, communication of findings to families and the interdisciplinary team, scheduling, documentation, and billing in accordance with CMS

Qualifications

• Associate's degree in Occupational Therapy.
• Must have active OTA licensure in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Execute treatment plans and follow goals set by supervising therapist.
• Interpret and communicate evaluation findings to residents, families, and team members.
• Establish individual resident therapy schedules with team members.
• Document and bill occupational therapy services as per plan of care.
• Perform all other duties as assigned to treat patients.
• Comply with CMS documentation standards and daily documentation.

Education

Associate's degree in Occupational Therapy

OTA licensure in state(s) of practice
