Job summary

Evolve Therapy Services in Minneapolis, MN is seeking an Occupational Therapist for an in-house Skilled Nursing Facility. This full-time position offers the opportunity to plan, deliver, and direct occupational therapy services in a facility setting.

Responsibilities include screening residents, developing treatment plans with goals, coordinating with the interdisciplinary team, documenting and billing, and supervising COTA as needed.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in the practicing state(s) is required.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Conduct regular screening of residents to determine best intervention or treatment for each patient.
• Evaluate residents to inform and develop treatment plans; establish short- and long-term goals and methods to achieve goals.
• Interpret and communicate evaluation findings, treatment plans, and recommendations to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules in cooperation with other team members.
• Conduct therapy according to established treatment plan and ensure proper documentation and billing of occupational therapy services.
• Perform all other duties as assigned and as required to effectively treat patients.
• Oversee COTA as needed and ensure documentation and treatments are carried out with clinical excellence and proper reimbursement practices.

Education

Degree in Occupational Therapy

Active Occupational Therapy license in practicing state(s)

Ongoing continuing education and professional development to maintain licensure
