Job summary

Community LIFE in McKeesport, PA is seeking a Certified Occupational Therapy Assistant (COTA) to serve as a Home Care Coordinator, helping older adults stay independent at home.

You will assess participant needs with an interdisciplinary team, coordinate home care services, support transitions from hospitals and rehab facilities, and help prevent unnecessary institutional care.

Qualifications

• Graduate of an accredited COTA program.
• Valid Pennsylvania Driver's License (local travel is required; mileage reimbursed).
• At least 1 year of experience with older adults in long-term care, home health, or community-based care preferred.

Responsibilities

• Assess participant needs with an interdisciplinary care team.
• Coordinate home care services and support hospital/rehabilitation transitions.
• Help prevent unnecessary institutional care and support home living.

Location: McKeesport, PA service area

Schedule: Monday-Friday, 7:30 AM-4:00 PM, plus one weekend day per month and on-call rotation every 5 weeks
