Job summary

Community LIFE is seeking a Certified Occupational Therapy Assistant (COTA) to serve as a Home Care Coordinator in McKeesport, PA. This role supports older adults to live safely at home and coordinates home care services.

You'll assess participant needs with an interdisciplinary team, coordinate services, support transitions from hospitals and rehabilitation facilities, and help prevent unnecessary institutional care.

Qualifications

• Graduate of an accredited COTA program.
• Valid Pennsylvania Driver's License (local travel is required).
• At least 1 year of experience with older adults in long-term care or home health preferred.

Responsibilities

• Assess participant needs with an interdisciplinary care team.
• Coordinate home care services and support transitions from hospitals/rehabilitation facilities.
• Help prevent unnecessary institutional care.

Location: McKeesport, PA service area

Schedule: Monday-Friday, 7:30 AM-4:00 PM, plus one weekend day per month and on-call rotation every 5 weeks
