Job summary

Bear Mountain is seeking a compassionate Certified Occupational Therapy Assistant to join our rehabilitation team. You’ll work under an Occupational Therapist to provide individualized therapy that supports resident goals and independence.

Responsibilities include implementing treatment plans, monitoring progress, documenting outcomes, and participating in care and discharge planning. A strong focus on resident-centered care, teamwork, and ongoing professional development drives our department.

Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current state licensure as a Certified Occupational Therapy Assistant (COTA).
  • Long-term care, skilled nursing, or rehabilitation experience preferred.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with therapists, nursing, and interdisciplinary teams.

Responsibilities

  • Provide occupational therapy treatments according to established plans of care.
  • Assist the Occupational Therapist in developing and implementing effective treatment programs.
  • Deliver individualized and group therapy interventions based on resident needs and goals.
  • Monitor resident progress and communicate outcomes with the Occupational Therapist and care team.
  • Complete accurate and timely documentation in accordance with state and federal regulations.
  • Participate in care planning, discharge planning, and interdisciplinary team meetings.
  • Educate residents, families, and caregivers on strategies to promote independence after discharge.
  • Maintain a safe, organized, and welcoming therapy environment.
  • Support quality improvement initiatives and department goals.
  • Participate in continuing education to maintain clinical competency.
