Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin is seeking a highly motivated Certified Occupational Therapy Assistant (COTA) to join our team on a per diem basis. You will assist licensed therapists, develop and implement ADL-focused treatment plans, and help residents regain independence through targeted activities and therapy sessions.

The ideal candidate holds Wisconsin COTA certification, a high school diploma (associate's degree preferred), and strong communication skills.

Qualifications

• Current COTA certification in Wisconsin required.
• High school diploma or equivalent; associate's degree preferred.
• Experience in long-term care or rehabilitation setting preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Assist OT in developing and implementing OT treatment plans.
• Conduct therapy sessions focusing on ADLs, mobility, and cognitive skills.
• Monitor and record patient progress with recommendations for adaptations.
• Collaborate with other healthcare professionals for comprehensive care.
• Participate in interdisciplinary team meetings to discuss patient plans.
