Job summary

Rock River Nursing & Rehab in Fort Atkinson, Wisconsin is seeking a highly motivated Certified Occupational Therapy Assistant (COTA) to join our team on a per diem basis. You will assist licensed therapists, develop and implement ADL-focused treatment plans, and help residents regain independence through targeted activities and therapy sessions.

The ideal candidate holds Wisconsin COTA certification, a high school diploma (associate's degree preferred), and strong communication skills.

Qualifications

• Current COTA certification in Wisconsin required.

• Experience in long-term care or rehabilitation setting preferred.

Job Summary

We are seeking a highly motivated and skilled Certified Occupational Therapy Assistant (COTA) to join our team at Rock River Nursing & Rehab in Fort Atkinson, Wisconsin. As a COTA, you will play a vital role in providing high-quality care to our residents, utilizing your expertise to develop and implement individualized occupational therapy plans. Our Per Diem COTA will work alongside our dedicated healthcare team to promote recovery, independence, and overall well-being.

Responsibilities

• Assist Occupational Therapists in the development and implementation of occupational therapy treatment plans

• Conduct therapy sessions with residents, focusing on activities of daily living (ADLs), mobility, and cognitive skills

• Monitor and record patient progress, providing recommendations for adaptations or modifications as needed

• Collaborate with other healthcare professionals to ensure comprehensive care and communication

• Participate in interdisciplinary team meetings to discuss patient care and treatment plans

Requirements

• Current certification as a Certified Occupational Therapy Assistant (COTA) in the state of Wisconsin

• High school diploma or equivalent required; associate's degree or higher in Occupational Therapy Assistant preferred
