Job summary

Qualifications

• Three years of experience working with hard-to-serve client populations in corrections or human services, with case management, assessment, and service coordination.
• Experience with offender management processes such as classification, programming, and parole planning is a plus.
• Note: entry-level corrections experience may not meet minimum qualifications as defined in the posting.

Responsibilities

• Perform initial assessments and admission data summaries covering criminal history, medical and social needs, education, and vocational interests.
• Access and enter data in CDOC information systems and external sources; analyze and link information.
• Meet with offenders to develop initial case management plans and coordinate services for housing, employment, education, and treatment.
• Coordinate custody placement decisions using CTAP criteria and CTAP-related requirements.
• Provide ongoing case management and re-assessment to support successful re-entry.
