Job summary

El Paso County is seeking a dedicated case manager to support inmates at the Criminal Justice Center with linking to behavioral health services and comprehensive discharge planning. You will coordinate housing, employment, medical, and education needs, and ensure continuity of care prior to release.

This role requires a Bachelor's degree in a human behavioral science or criminal justice field and at least one year of related experience.

Qualifications

• Knowledge of case management and community mental health and substance abuse services.
• Ability to exercise discretion and maintain confidentiality.
• Ability to assess situations and make prudent decisions; conflict resolution and problem-solving.
• Ability to complete paperwork, reports, and documentation.
• Ability to plan, schedule, and organize.
• Effective communication with coworkers, inmates, other agencies, and the public.
• Proficiency in Microsoft Office (Word, Excel, Outlook).
• Maintain regular and punctual attendance.

Responsibilities

• Provides case management to inmates in a pre-sentence status at CJC.
• Connects inmates to community-based behavioral health support to reduce recidivism.
• Creates discharge and transition plans for housing, employment, medical, education, therapy, and public services.
• Coordinates appointments with probation and/or parole officers and community partners.
• Completes comprehensive discharge paperwork for releasing inmates.
• Serves as post-release contact and monitors aftercare assistance.
• Updates client database with services and outcomes; generates reports.
• Offers skill-based psychoeducation trainings as needed.
• Communicates with BRIDGES program to assist inmates in connecting with behavioral healthcare.
• Coordinates with DHS to verify Medicaid status and with jail medical vendor for MAT referrals.
• Performs other duties as required.
