Job summary

Responsibilities include guiding patients through admission, discharge, and post-discharge care, educating on inhaler use, arranging pulmonary rehab and PFTs, and contributing to quality improvement and care

Responsibilities

• Following all COPD patients during their admissions addressing the coordination of care and promoting self-management to decrease rehospitalization.
• Educates and ensures patient/family understanding of symptoms, plans, medications, and discharges.
• Collaborates with the care team to determine functional deficits and areas for improvement.
• For patients nearing discharge, confirms education, medication changes, discharge planning, and DME needs.
• Support post-discharge care coordination with follow-up calls and scheduling of PFTs, rehab, and preventive screenings.
