Job summary

Americare in Brooklyn, NY seeks a Visiting Nurse Coordinator of Care to manage all aspects of home-based patient care. The role involves conducting home visits, completing documentation, and coordinating with the primary care physician to ensure seamless services.

This full-time weekend schedule runs Friday, Saturday, and Monday. The Nurse will work independently with strong clinical skills and a valid NYS RN license, and will receive comprehensive benefits and ongoing support from a dedicated
