Job summary

The Office of Community Safety in Prince William County is seeking a Community Navigation Specialist to coordinate the Coordinated Community Cares (CCC) program with Fire & Rescue, aging, social services, and healthcare partners. You will lead multidisciplinary teams, manage case loads, track outcomes, and prepare reports to inform planning.

A high school diploma and five years of related experience are required; a Bachelor's is preferred.

Qualifications

High school diploma or G.E.D. is required.
Minimum of five (5) years of progressively responsible experience in case management, community navigation, human services, public health, behavioral health, emergency response, social services, or related field.
Bachelor's degree in Social Work, Human Services, Public Administration, Public Health, Criminal Justice, Behavioral Health, or related field preferred.

Responsibilities

Coordinate services for residents with complex needs within CCC program.
Facilitate multidisciplinary team meetings and cross-agency collaboration.
Track resident outcomes and identify gaps in service delivery.
Lead administrator for the case management system and ensure data collection and reporting.
Support strategic planning and data-driven decision making.
