Job summary

Commonwealth Catholic Charities (CCC) seeks a dedicated professional to provide comprehensive services to individuals and families experiencing homelessness across Virginia. You will conduct intakes, coordinate housing referrals, and connect clients with health, social, and financial supports.

Emphasis on ending homelessness and serving vulnerable communities. The role requires a Bachelor’s Degree with related experience and a valid driver’s license.

Qualifications

• Bachelor’s Degree required with two years of related experience.
• Persons with lived experience of homelessness encouraged to apply.
• Must have a valid driver’s license with three years clean driving record.

Responsibilities

• Engage individuals experiencing homelessness and assist them with accessing health, housing, and social services.
• Provide in-person, community-based assistance and connect with individuals in their location.
• Assist eligible individuals/households in applying for mainstream benefits including Medicaid, SNAP, Social Security disability.
• Assist individuals with connecting to basic needs such as food, clothing, transportation, and identification.
• Conduct client intakes and assess needs to refer to housing services and shelter.
• Maintain client records in HCIS (Service Point) with demographics and service transactions.
• Advocate on behalf of clients to obtain essential services.
• Collaborate with community agencies to address barriers and complex needs.
• Identify resources and provide referrals to housing, medical, and mental health services.
• Assist with crisis intervention and liaison between hospitals and community agencies.
• Provide case management to a core caseload and coordinate services.
• Perform street outreach to connect individuals to shelter and housing.
• Consult with mental health professionals for screening and coordination.
• Develop positive relationships with partners to improve services.
• Staff on call hours at Housing Resource Center or Youth Hub.
• Represent the Agency professionally and ethically.
• Demonstrate sensitivity to cultural and socioeconomic characteristics.
• Demonstrate strong interpersonal, decision-making, problem-solving, and communication skills.
