Job summary

Prince William County Government is seeking a Community Navigation Specialist to coordinate the Coordinated Community Cares (CCC) program within the Office of Community Safety. You will work with Fire & Rescue, the Area Agency on Aging, the Department of Social Services, and other partners to connect residents with services while strengthening collaboration.

The ideal candidate brings experience in case management, care navigation, and data-driven reporting.

Qualifications

• High school diploma or equivalent required; 5+ years in case management or related field preferred.
• Bachelor's degree preferred in Social Work, Human Services, Public Health, Public Administration, Criminal Justice, Behavioral Health, or related field.

Responsibilities

• Coordinate services for residents with complex needs across multiple agencies.
• Lead multidisciplinary team meetings and support coordinated case management.
• Track resident outcomes and identify gaps in service delivery; prepare reports and program evaluations.
