Job summary

Cityblock Health in Maryland is seeking a Community Health Partner (CHP) to work with RN Care Managers and support integrated social care coordination for members with complex needs. You will visit members in the community or home, assist with navigating social services, and coordinate care with internal and external providers.

Requires 2+ years in care coordination, a valid driver's license, and a Bachelor's degree.

Qualifications

• 2+ years of progressive experience in care coordination, community health, or related field.
• Experience with high-volume caseload.
• Unrestricted driver's license and vehicle for daily use.

Responsibilities

• Engage with members from care teams and explain program expectations and goals.
• Complete assessments and screenings; collaborate with RN Care Manager on placement needs.
• Develop and update care plans with RN Care Manager and address social needs.
• Follow up with members on care coordination, benefits, and education.
• Meet members in the community (home, SNF, shelter, hospital) as needed.
• Use care facilitation and EHR platforms to document interactions and manage tasks.
