Job summary

Kids for the Future is seeking an Assessor in Ohio to perform person-centered assessments and determine eligibility for programs and services. You will educate clients and caregivers about available resources and support their understanding of service rationale.

The role involves developing initial care plans, assisting with discharge planning, and maintaining compliant electronic records. Travel within Butler, Clermont, Clinton, Hamilton and Warren Counties is required.

Qualifications

Must hold an Ohio license as Social Worker, LPN, or RN.
Minimum of 1 year in medical social work, home health care, or geriatrics.
Experience with electronic documentation is a plus.
Strong knowledge of issues related to the elderly population.
Proficient computer skills including Windows, MS Word and MS Excel.
Valid driver's license and reliable transportation for travel within counties.
Ability to travel within Ohio for training.

Responsibilities

Conduct person-centered assessments to determine eligibility for programs and services.
Develop service plans with clients and caregivers using Service Guidelines.
Assist with discharge planning and transitions between care settings.
Provide education and support to individuals, caregivers, and legal representatives about resources and eligibility.
Maintain accurate and timely documentation in electronic systems and ensure HIPAA compliance.
