Job summary

Central Clinic Behavioral Health is seeking an Assessor for the CDC Behavioral Health/Employment Success Program in Hamilton, Ohio. This full-time position focuses on performing diagnostic assessments and developing employment-related treatment plans for clients with mental health and substance use concerns.

Qualifications

• Master's degree in Social Work, Counseling, or related field.
• Ohio licensure in social work or counseling.
• SUD experience preferred.

Responsibilities

• Enter clients into the EHR and maintain data spreadsheets.
• Administer drug screens as needed.
• Provide diagnostic assessments to address barriers to employment.
• Develop a treatment plan and link clients to appropriate providers.
• Staff cases with JFS and other providers as needed.
• Provide direct case management and crisis intervention as required.
• Collaborate with other agencies involved in client care.

Position Summary

• Participate in all agency and/or Butler County Job Department of Job and Family Services committees of projects as designated.
• Assist with entering clients into the EHR, maintain data spreadsheets, administer drug screens as needed.
• Provide diagnostic assessments to address barriers to employment for unemployed or underemployed clients that may have mental health and/or co-occuring disorders of mental health and substance abuse.
• Develop a treatment plan to address identified barriers to employment and assist in connecting clients to the correct providers to address these issues.
• Staff cases as needed with JFS and other providers involved in client's case as needed.
• Submit necessary documentation in a timely manner; meet service delivery targets.
• Participate in training to strengthen effective delivery of services.
• Provide direct case management services; these include: assessment, treatment services planning, and assistance in daily living, supportive services, crisis intervention, linkage/referrala/advocacy, and suport of client's self-determination.
• Work collaboratively with other agencies involved in client's care.
• Assume other duties as required by Supervisor.
