Job summary

COA is seeking PRN Licensed Social Workers and Registered Nurses to conduct person-centered assessments across Southwestern Ohio, including Blue Ash training. You’ll help older adults stay independent by coordinating services, educating clients, and advocating for safety and needs.

Role offers flexibility with daytime hours and paid per assessment. Candidates should have active Ohio LSW or RN licenses, 1+ year in relevant fields, and strong communication and empathy.

Qualifications

• Active Ohio LSW or RN license in good standing.
• At least 1 year of experience in medical social work, home health, or geriatrics.
• Strong understanding of aging-related issues and community resources.

Responsibilities

• Conduct telephonic and in-person assessments to determine program eligibility.
• Coordinate wraparound services for individuals with complex medical, behavioral, or social needs.
• Educate clients and caregivers on available resources and care planning.
• Advocate for clients facing health and safety risks or potential institutionalization.
• Collaborate with internal teams, healthcare providers, and community partners.
