Position Name: Hospice Registered Nurse Case Manager

Pay Rate:Range $2040 to $2160 Weekly Gross pay based on a 40 hour work week (Includes non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Unit: Home setting

Shift timings: 8-hour Days, 8.00 am to 5.00 pm

Schedule: 40 hours/week guaranteed. 5 X 8-hrs, 5 shifts per week, Mon-Fri with some on-call requirements

Position Requirements:

• Registered Nurse: 2years - Required
  
• Recent/Current Hospice nursing experience in a home health setting: 1 year within the last 3 years – Required
• Hands on Home Care Home Base(HCHB) charting experience – Required
  
• Must have validDriver’s License and Personal Auto Insurance

Job Description:

• Provide and coordinate care for assigned Hospice patients

• Provide Hospice nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
  
• Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
  
• Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
