Job summary

Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse for a travel/contract position in Duluth, GA. This full-time role offers 40 hours per week with travel/housing stipends for eligible travelers.

Responsibilities include providing hospice nursing care at patients' homes, developing care plans, and documenting visits within 24 hours. RN license in state/compact is required; ASN is required; BSN preferred; BLS; valid driver’s license and auto insurance.

Qualifications

• 1 year RN experience required.
• Hospice/home health experience in last 3 years preferred.
• HCHB charting experience required.

Responsibilities

• Provide and coordinate care for hospice patients at home.
• Develop and implement nursing care plans including Start of Care and admissions.
• Support patient recovery to stay at home with family.
• Educate patients and families on disease management and goals.
• Deliver hospice nursing care within state nursing practice standards.
• Ensure physicians orders are legible and transcribed correctly.
• Document all patient visits within 24 hours.
• Provide bereavement support to families as needed.

MileageReimbursement: $0.49/mile for distancecovered between patient home appointments

Schedule: 40 hours/week guaranteed, 5 shifts per week, Mon-Fri

Position Requirements:

• Registered Nurse: 1 year - Required
  
• Recent/CurrentHospice Nursing experience in a home health setting: 1 year within the last 3 years – Required
  
• Home Care Home Base(HCHB) charting - Required
  
• Must have validDriver’s License and Personal Auto Insurance
  
• Must be CovidVaccinated or provide medical or religious exemption

• Ensure physicians’orders are legible if taken verbally and transcribed appropriately perpolicy and procedure.
  
• Ensure patients’status is assessed on an ongoing basis and pertinent information isgathered and documented.
  
• Notify physicians ofsignificant changes in status.
  
• Document and completecharting for all patient visits within 24 hours.
