Job summary

Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse Case Manager for a 13-week travel/contract position in West Plains, MO. Start ASAP. 40 hours/week, 5 shifts, Mon-Fri, with travel/housing stipend for eligible travelers.

RN with hospice/home health background, ASN required (BSN preferred); valid RN license and BLS required. Territory includes West Plains and possibly Mountain Grove, MO.

Qualifications

• Registered Nurse: 1 year - Required
• Hospice Nursing experience in a home health setting: 1 year within the last 3 years – Required
• HCHB charting - Highly Preferred
• OASIS experience - Preferred
• Must have validDriver’s License and Personal Auto Insurance

Responsibilities

• Provide and coordinate care for assigned patients in the comfort of their homes.
• Manage assigned patient caseload
• Develop and implement nursing care plans, including Start of Care and admissions, as assigned.
• Support patient comfort and dignity for terminally ill patients
• Empower patients and families to reach healthcare goals by educating them on disease management.
• Provide Hospice nursing care and services within nursing practice standards and protocols.
• Ensure physicians’ orders are legible and transcribed per policy.
• Ensure patient status is assessed and documented.
• Notify physicians of significant changes in status.
• Document and complete charting for all patient visits within 24 hours.
• Provide bereavement support to patients’ families as needed.
• Territories to cover: West Plains, MO and possibly Mountain Grove, MO
