Responsibilities include completing nutrition assessments, documenting with the Nutrition Care Process in the EMR, and collaborating with the interdisciplinary care plan team to optimize resident outcomes.

Qualifications

• Credentialed by the CDR is required.
• Dietetic licensure is required.
• Demonstrates excellent communication and client-centered care.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
