Job summary

Nutrition that Works, LLC is seeking a consultan t dietitian to support our long-term care community in Long Beach, CA. The role is 16 hours per week with flexible scheduling and competitive pay.

The dietitian will complete nutrition assessments, document with the Nutrition Care Process in the EMR, and work closely with the care team to optimize patient outcomes. Qualified candidates are credentialed by the CDR, hold a current dietetic license, and demonstrate strong communication skills.

Qualifications

• Credentials required by the CDR and current dietetic licensure.
• Client-centered approach with strong communication skills.
• Ability to work collaboratively in an interdisciplinary care team.
• Experience with nutrition care plan documentation.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR system.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
