Pleasant Hill Post Acute in Pleasant Hill, CA is seeking an experienced Licensed Vocational Nurse (LVN) Case Manager to coordinate resident care, oversee admissions and discharges, and advocate for residents and families.

In this full-time role, you will work with residents, families, physicians, nursing staff, rehabilitation teams, and payers to develop individualized plans, monitor status, and ensure timely, cost-effective services.

Qualifications

• Active LVN/LPN license is required.
• Experience in case management or nursing coordination is preferred.
• Ability to coordinate and document care plans across multidisciplinary teams.

Responsabilités

• Coordinate and implement individualized resident care plans with residents, families, physicians, nursing staff, and rehabilitation teams.
• Communicate plans of care and treatment protocols to residents and families.
• Participate in daily interdisciplinary meetings to coordinate resident care.
• Oversee admissions, discharges, and transitions of care.
• Perform utilization review to ensure appropriate and cost-effective care.
• Maintain accurate clinical documentation and required paperwork.
• Promote collaboration among residents, families, and care teams.
