Qualifications

• Minimum education is a high school diploma or equivalent; graduation from an accredited LVN program.

Responsibilities

• Support concurrent review and inpatient/post-acute utilization monitoring.
• Assist with discharge planning and transitions of care.
• Coordinate services across hospitals, SNFs, IRFs, LTACHs, and home health.
