Job summary

Boston Medical Center's Complex Care Management program places RN-led teams within primary care practices. Nurses collaborate with PCPs, behavioral health, pharmacists, and community resources to develop and execute patient-centered care plans addressing chronic disease and social determinants of health.

The role emphasizes motivational interviewing, careful assessment, and coordination with community partners to improve outcomes and reduce unnecessary healthcare utilization.

Qualifications

• Nursing degree required or equivalent; BSN preferred within 4 years.
• CCM Certification preferred or active RN licensure in MA.
• Experience in care management and vulnerable populations is preferred.

Responsibilities

• Identify and recruit appropriate patients for care management from lists and referrals.
• Conduct comprehensive bio-psycho-social-spiritual assessments and develop patient-centered care plans.
• Coordinate with community partners and care team to implement and monitor plans.
• Maintain HIPAA compliance and document care activities.
• Engage in interdisciplinary rounds and site operations to support program growth.
