Job summary

A healthcare recruiting agency is looking for a Complex Care Manager (RN) in Boston, MA. The successful candidate will collaborate with providers to identify patients for care management while implementing strategies that promote patient wellness. Candidates must have a Bachelor's in Nursing, at least three years of related experience, and a Massachusetts RN license. This full-time position includes a Monday to Friday schedule, excellent compensation, and benefits.

Qualifications

• At least three years of experience preferably in an ambulatory setting with Family Practice focus.
• Massachusetts Nursing license – RN required.
• Experience with telephone assessment and crisis management preferred.
• Excellent communication and documentation skills are a must.

Responsibilities

• Collaborates with providers and practice staff in identifying patients for care management.
• Formulates and implements care management plans to address patient needs.
• Establishes mutually agreed-upon care management plans with health care teams.
• Educates patients/families on choices available to them.
• Develops collaborative relationships with case managers and negotiates for patient services.
• Serves as primary contact for case management with physician organization.
