Job summary

The South Carolina Department of Behavioral Health and Developmental Disabilities, Office of Intellectual and Developmental Disabilities, seeks a Complex Care Coordinator to manage cases for individuals with complex needs, including those in hospital settings and those with DSS involvement.

Responsibilities include coordinating services, documenting activities, and facilitating transitions to other providers.

Qualifications

• Bachelor's or graduate degree from an accredited college or university and at least one year of experience with the target population.
• Knowledge of interagency case management principles and practices.
• Familiar with OIDD populations (intellectual/developmental disabilities, brain injury, spinal cord injury) and available services.
• Excellent verbal and written communication skills.
• Computer literacy and ability to use standard office software and electronic documentation systems.

Responsibilities

• Provide comprehensive case management services for assigned cases.
• Assist with implementing needed services and supports for stabilization.
• Authorize services as approved on the Support Plan.
• Facilitate transfer to identified case management provider after stabilization.
• Document activities and provide updates to supervisor.
• Perform other case management duties as assigned.
