Job summary

HCA Healthcare seeks a New Hampshire Market Complex Care Coordinator to support patient-centered care across our market, focusing on high-risk and medically complex patients in the ED and inpatient settings. You will collaborate with multidisciplinary teams to improve transitions, discharge safety, and throughput, serving as a market-wide resource for complex case management.

The role requires up to 75% travel to on-site meetings at market hospitals in Portsmouth, Derry, Manchester, and

Qualifications

ADN or BSN required, plus applicable social work licensure per NH rules.
Two years of acute care case management or care coordination experience.
RN license in NH or SW licensure preferred depending on track.
Case management certification preferred to support practice.

Responsibilities

Coordinate care for medically complex patients in ED and inpatient settings.
Facilitate patient throughput, safe discharge planning, and transitions of care.
Collaborate with physicians, nurses, social work, and ancillary staff.
Refer to payer programs, disease management, and community resources.
Document interventions, discharge plans, and case management activities.
Identify barriers to reduce LOS, ED boarding, and avoidable admissions.
Work with leadership to identify throughput improvement opportunities.
Review complex cases and develop action plans for timely progression.
Monitor trends in care coordination and discharge planning for quality
Build relationships with post-acute providers and community partners.
