Qualifications
- ADN or BSN required, plus applicable social work licensure per NH rules.
- Two years of acute care case management or care coordination experience.
- RN license in NH or SW licensure preferred depending on track.
- Case management certification preferred to support practice.
Responsibilities
- Coordinate care for medically complex patients in ED and inpatient settings.
- Facilitate patient throughput, safe discharge planning, and transitions of care.
- Collaborate with physicians, nurses, social work, and ancillary staff.
- Refer to payer programs, disease management, and community resources.
- Document interventions, discharge plans, and case management activities.
- Identify barriers to reduce LOS, ED boarding, and avoidable admissions.
- Work with leadership to identify throughput improvement opportunities.
- Review complex cases and develop action plans for timely progression.
- Monitor trends in care coordination and discharge planning for quality
- Build relationships with post-acute providers and community partners.

\The New Hampshire Market Complex Care Coordinator is responsible for supporting patient-centered care coordination across the market continuum, with a primary focus on high-risk, medically complex, and socially vulnerable patient populations presenting in the Emergency Department and inpatient settings. This role collaborates with multidisciplinary teams to facilitate timely patient progression, reduce avoidable delays in care, improve transitions of care, and support safe discharge planning. The coordinator serves as a market-wide resource for complex case management, length-of-stay reduction initiatives, community partnership development, and care transition optimization. The NH Market Complex Care Coordinator supports strategic initiatives focused on emergency department throughput, inpatient progression of care, extended length-of-stay management, avoidable admission reduction, and operational excellence across the market.\\
\\Eligible candidates must reside in our New Hampshire market\ - position requires up to 75% travel to on-site meetings several times per week at our market hospitals: Portsmouth Regional Hospital (Portsmouth), Parkland Medical Center (Derry), Catholic Medical Center (Manchester) and Frisbie Memorial Hospital (Rochester).\

- Coordinates care for medically complex, chronically ill, and socially vulnerable patients in both Emergency Department and inpatient environments.\ \
- Facilitates patient throughput with an ongoing focus on quality outcomes, efficiency, and safe care transitions.\ \
- Collaborates with physicians, nursing, case management, social work, and ancillary departments to support patient progression and discharge planning.\ \
- Makes referrals to payer programs, disease management programs, community agencies, and other resources as appropriate.\ \
- Documents professional recommendations, care coordination interventions, discharge plans, and case management activities.\ \
- Identifies and addresses barriers contributing to extended length of stay, emergency department boarding, avoidable admissions, and delayed discharges.\ \
- Collaborates with facility and market leadership to identify opportunities for throughput improvement and reduction in length of stay.\ \
- Reviews complex cases and develops action plans to facilitate timely progression of care and discharge.\ \
- Monitors trends related to care coordination, discharge planning, and patient flow to identify process improvement opportunities.\ \
- Develops and maintains relationships with post-acute providers, skilled nursing facilities, inpatient rehabilitation facilities, home health agencies, community organizations, and other external partners.\ \
- Serves as a liaison between hospitals and community resources to improve access to services and facilitate seamless transitions of care.\ \
- Identifies gaps in community resources and collaborates with stakeholders to develop solutions that support patient needs.\ \
Associate Degree in Nursing (ADN) or\
\\Required\\
\Bachelor’s Degree in Social Work (BSW) or\
\\Required\\
\Master’s Degree in Social Work, Healthcare Administration, Public Health, or other healthcare-related field\
\\Required\\
\Two (2) years of case management experience in an acute care, emergency department, care coordination, utilization management, social work, or population health setting.\
\\Preferred\\
\Experience working with complex patient populations\
\\Preferred\\
\Experience in emergency department case management and care coordination\
\\Preferred\\
\Experience collaborating with community resource organizations\
\\Preferred\\
\Experience in discharge planning and transitions of care\

\Currently licensed as a Registered Nurse (RN) in New Hampshire or applicable social work licensure in accordance with NH state regulations\

\Travel required – job may require up to 75% travel.\
