Job summary

Freedom Home Health and Hospice Care Services Inc. seeks a Speech-Language Pathologist to implement standards of care for speech-language pathology services within the home health setting.

You will evaluate patients, design plans of care, administer therapy, and coordinate with physicians to ensure comprehensive treatment. The role requires a master’s or doctoral degree in SLP, current CPR certification, licensure/eligibility, and at least one year of clinical experience.

Qualifications

• Master's or doctoral degree in speech-language pathology (SLP).
• CPR certification and valid state licensure/ eligibility required.
• Minimum one year of clinical SLP experience.
• Must have a valid driver’s license and reliable vehicle.

Responsibilities

• Conducts appropriate evaluations, assesses home environment and equipment needs related to speech/language function.
• Identifies functional speech defects and plans care to improve patient function.
• Administers speech therapy program using specialized techniques and equipment.
• Recommends communication devices/aids as indicated.
• Confers with referring physician and other staff to ensure coordinated care.
• Participates in case conferences, inservices and other required meetings.
• Prepares and submits notes per policy.
• Maintains clinical competency in speech-language pathology.
• Identifies patient/family needs for other services and refers as appropriate.
• Instructs patient, family/caregiver and staff in treatment regimen.
• May supervise home health aide in speech-language therapy procedures.
• Performs other duties to facilitate service delivery.
