Job summary

Compassus is seeking a Home Health Speech Language Pathologist - PPV PRN to deliver specialized care for patients with speech, language, and swallowing disorders in a home health setting. You will collaborate with physicians, nurses, families, and the care team to implement the plan of care and ensure quality outcomes.

The role requires a Bachelor's degree in Speech Language Pathology (Master's preferred), an active license, and at least one year of home health experience.

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting required.

Responsibilities

• Provides care using infection control measures per OSHA requirements.
• Communicates effectively with patients, families, and care teams to coordinate services.
• Performs initial and ongoing assessment of speech, language, cognitive, and swallowing disorders.
• Develops and revises the Plan of Care with the physician and care team.
• Supervises Home Health Aides to ensure quality and continuity of service.
