Job summary

Compassus is seeking a Speech Language Pathologist to provide home health services under physician direction within the Home Therapy Program, with family and nurse involvement as needed. The role includes ongoing comprehensive assessment (OASIS/Agency), developing plans of care with the physician and care team, and delivering speech therapy services and patient education.

Bachelor’s in SLP is required; Master’s preferred; minimum one year home health experience; adult/geriatric care experience

Qualifications

• Bachelor's degree in Speech Language Pathology required.
• Master's degree in Speech Language Pathology preferred.
• Minimum of one year of experience in a home health setting required.
• Experience working with adult/geriatric patients required.

Responsibilities

• Provides care with infection control to protect staff and patient.
• Communicates with patient, family, and team to coordinate services.
• Accepts clinical assignments appropriate to education and competence.
• Monitors cases for third-party payor requirements.
• Provides speech therapy services and education on disease processes.
• Performs initial and ongoing comprehensive assessment (OASIS/Agency).
• Develops and revises Plan of Care with physician and care team.
