Job summary

You will advocate for client rights and coordinate services with healthcare providers and community resources. Qualified candidates hold an MSW; licensure (LMSW/LCSW) is preferred but not mandatory depending on the position.

Qualifications

• Master’s degree in Social Work (MSW) from an accredited program required.
• LMSW or LCSW preferred but not mandatory depending on role.
• Strong interpersonal and written communication skills required.

Responsibilities

• Conduct comprehensive assessments of clients' needs, strengths, and challenges.
• Develop individualized service plans addressing client needs.
• Provide counseling support for mental health, family dynamics, crisis intervention, or substance use.
• Advocate for clients’ rights within healthcare settings and community resources.
• Coordinate services with healthcare providers, schools, and community organizations.
• Maintain accurate records of client interactions in compliance with ethical and legal standards.
• Stay informed about local resources to provide appropriate referrals.
• Participate in multidisciplinary team meetings to discuss patient care strategies.
