Job summary

ABL Health Care LLC in Milpitas, CA is seeking a dedicated Registered Nurse (RN) for full-time home health. You will evaluate patients, develop care plans, and coordinate with the clinical team while delivering compassionate, patient-centered care in the home setting.

This role requires California RN licensure, home health or hospice experience, and a valid driver's license. You will travel to patients' homes and use company devices to document care and communicate with the Interdisciplinary

Qualifications

• Current Registered Nurse license to practice in California (Required).
• Experience in Home Health or Hospice care.
• Valid California Driver's License.
• Ability to travel to patient homes.

Responsibilities

• Provide nursing care within the scope of practice per patient care plan, in coordination with the Interdisciplinary Team, in home health settings.
• Administer treatments, monitor patient conditions, and report changes to the Physician, Case Manager, and Director of Nursing as needed.
• Maintain accurate clinical records and submit documentation using company devices in a timely manner.
• Supervise Licensed Vocational Nurses (LVNs) when assigned.
• Comply with all company policies and procedures to ensure high standards of care.
