Job summary

AHRC New York City is seeking two part-time registered nurses for our Queens ICF residence to monitor health needs and promote wellness. The RN will ensure nursing care complies with federal/state regulations and agency policy, working a day shift at 89-02 162nd Street, Jamaica, NY.

Responsibilities include coordinating medical care, observing patient status, communicating with physicians and families, and updating nursing plans. Shift options include day and overnight with no benefits.

Responsibilities

• Coordinate medical care for assigned individuals and arrange follow-up care as needed.
• Assess consumer health status through direct observation and document findings.
• Communicate with physicians, families, and clinicians involved in services.
• Notify team about hospitalizations, ER visits, injuries or illnesses as required.
• Accompany individuals to medical appointments to advocate for health issues.
• Attend management, staff, families and clinicians meetings to discuss health concerns.
• Develop and update individual nursing plans and protocols reflecting changes in status.
• Supervise AMAP administration of medications and treatments as prescribed.
• Review MARs and ensure proper medication storage and disposal procedures.
• Monitor health goals and nursing plans reflecting consumer status.
• Monitor dietary and nutritional needs and oversee meal times.
• Participate in discharge planning and referrals to other agencies.
• Write progress notes and nursing assessments for physician review.
• Ensure referrals for routine and special medical services are followed up.
• In compliance with OSHA, monitor infection control and environmental guidelines.
