Job summary

Paylocity is seeking an RN Case Manager to provide skilled nursing care to terminally ill patients based on ongoing assessments. The role plans, implements, and evaluates nursing care through systematic assessment and coordination with the care team.

The position requires an accredited nursing program graduate with an active multi-state RN license in Kentucky or Ohio, at least one year of patient-care experience, strong communication, and the ability to work independently.

Qualifications

• RN license in Kentucky or Ohio is required.
• Minimum 1 year of patient-care experience in a hospital, nursing home, or related settings preferred.
• Strong empathy, responsibility, dependability, and teaching ability.

Responsibilities

• Perform assessments of patient needs using clinical tools and data.
• Create individualized case management and treatment plans.
• Educate patients and caregivers on benefits and resources.
• Document complex medical information to guide care and outcomes.
• Follow case and quality management processes per regulations and company procedures.
• Maintain strong written and verbal communication with patients, physicians, and stakeholders.
• Respond promptly to patient and family needs.
• Establish and maintain professional relationships with patients, families, and providers.
• Provide care during on-call hours as scheduled.
• Work Monday–Friday, 8:00 am–4:30 pm, with weekend rotation as scheduled.
