Job summary

Millenia Hospice in Fort Worth, Texas is seeking a full-time RN Case Manager to serve patients and families in the assigned territory. This field-based role requires compassionate, skilled nursing care and coordination.

You will develop plans of care, manage symptoms, educate families, and collaborate with an interdisciplinary team to ensure comfort, dignity, and quality of life for patients at end of life.

Qualifications

• Active Texas RN license.
• Current CPR certification.
• Valid driver’s license with reliable transportation and ability to travel throughout the territory.
• Strong clinical assessment and care coordination abilities.
• Excellent communication and organizational skills.
• Ability to manage a field-based patient caseload independently and maintain documentation.

Responsibilities

• Conduct comprehensive initial and ongoing nursing assessments.
• Develop and update Plans of Care based on patient conditions and end-of-life goals.
• Provide skilled nursing care and symptom management.
• Monitor patient changes and respond to urgent concerns.
• Coordinate care with physicians, social workers, chaplains, and other team members.
• Educate patients and families about disease progression, medications, and caregiving techniques.
• Maintain timely clinical documentation in the EMR.
• Participate in Interdisciplinary Group meetings and on-call rotation.
• Ensure compliance with hospice regulations and company policies.
