Qualifications

• Graduated from an accredited nursing program with Georgia RN licensure.
• Experience in hospice/home health care preferred.
• Strong patient education and family engagement skills.
• Ability to work independently in a home-based setting.

Responsibilities

• Develop and implement individualized plans of care with interdisciplinary team.
• Complete and submit OASIS documents and visit notes.
• Provide patient and family education on disease processes and self-management.
• Supervise LPNs and HHAs per VNHS policy.
• Schedule patient visits and manage caseload efficiently.
