Job summary

Ohio Living seeks a Registered Nurse to provide in-home care and support across Ohio health systems. You will assess patient needs, develop care plans, and coordinate with providers to ensure high-quality outcomes.

The role requires one year of acute care or medical-surgical nursing experience, Ohio RN license, and a driver’s license. Proficiency with Windows and EMR is preferred; weekend/evening shifts and on-call duties may be required.

Qualifications

Current unencumbered Ohio RN license to practice as an RN.
One year experience in acute care or medical-surgical nursing.
Experience in a Medicare-certified home health agency preferred.
Proficiency with Windows, Microsoft Office (Word, Excel, PowerPoint), and the internet.
Proficiency with EMR preferred.
Must read, write, speak, and understand English.

Responsibilities

Provides ongoing assessment and nursing care to patients at homes and health care facilities and develops a comprehensive care plan.
Follows current nursing standards of practice and agency protocols for home health.
Provides quality plan and symptom management for patient comfort.
Communicates with other health care providers about changes in condition and patient needs.
Oversees the implementation of the patient’s plan of care and evaluates effectiveness.
All other duties as assigned.
