Job summary

Otterbein SeniorLife is hiring a Registered Nurse (RN) – Case Manager for Home Health in the Lebanon, Ohio area. You will perform comprehensive home health nursing care, assess patient needs, and develop individualized care plans in collaboration with families and physicians.

Qualified candidates hold an Ohio RN license and a Bachelor’s in Nursing, with at least 1 year of clinical nursing experience; Home Health or Hospice experience is preferred.

Qualifications

• Bachelor’s degree in Nursing; valid Ohio RN license; CPR certification.
• Experience: minimum 1 year clinical nursing; Home Health or community health experience preferred.
• Must have valid driver’s license with reliable vehicle and clean driving record.

Responsibilities

• Complete initial patient and family assessment to determine home care needs.
• Develop and revise the plan of care; set goals with patient/family involvement.
• Manage nursing diagnoses and provide therapeutic, preventive, and rehabilitative care.
• Administer medications and treatments as prescribed; monitor patient status.
• Coordinate discharge planning and communicate with physicians and care team.
• Instruct, supervise, and evaluate home health aides; ensure equipment and services are arranged.
• Participate in on-call duties as required by policy.
