Ohio Living seeks an RN Field Staff - Hospice to plan, coordinate, and deliver quality hospice care in patients' homes and care facilities across Ohio. The role emphasizes care management, symptom control, and collaboration with other health care professionals.

The ideal candidate holds an Ohio RN license, has at least 1 year of acute care/med-surg experience, and is skilled with EMR, Windows, and MS Office. Home health experience is preferred; weekend/evening shifts and on-call are required.

Qualifications

• Current unencumbered Ohio RN license.
• At least 1 year of acute care or medical-surgical nursing experience.
• Medicare-certified home health agency experience preferred.
• Proficient with Windows, Microsoft Office, and EMR systems.

Responsibilities

• Care Management and Service Delivery – 90%: assess and provide nursing care at patients' homes or facilities and develop comprehensive care plans.
• Deliver effective pain and symptom management and educate patients and caregivers.
• Communicate changes in condition with other health care providers and participate in IDG/care planning.
• Ensure quality and compliance with regulatory guidelines and chart accurately.
