Job summary
Kansas City Hospice & Palliative Care is seeking a compassionate Registered Nurse (RN) Case Manager to join an interdisciplinary team in Kansas City, MO. The role focuses on delivering expert hospice care, educating families, and supporting patients with dignity and compassion.
Requirements include MO/KS RN licensure (or compact), CPR certification, and at least 1 year of hospice bedside experience. Excellent communication and organizational skills are essential, with a valid driver’s license
Qualifications
- Valid, unencumbered Missouri and Kansas RN license (or compact).
- Minimum of one year bedside hospice experience preferred.
- Ability to communicate effectively in oral and written formats.
- CPR certification required.
- Valid driver's license and reliable transportation.
Responsibilities
- Provide high-quality hospice nursing care and patient education.
- Collaborate with an interdisciplinary team to support patients and families.
- Documentation of visits and timely communication with care teams.
- Assist families with care plans and symptom management.
